Transitional Living Facility
The Transitional Living Facility/Program is designed to provide additional support to persons transitioning from a higher level of care, experiencing acute difficulty with symptoms of their illness or needing additional supports in order to reside in the community. The program is located within an assisted living facility and includes 2FTE staff who provide additional support, assessment and rehabilitation during the hours of 1-9:30 PM daily.
The program includes daily contact, groups, leisure activities and the availability of staff for needed additional supports. Linkage to services and coordination with other care providers is a strong component of this program. For more information, call 441-8344.
