Harvest Project

Older persons who suffer the combined effects of impaired health, mental illness, and/or substance abuse pose difficult challenges to families, professional, and our community. Perplexed families don't know where to turn for help, what to do, or what to expect.

The Harvest Project offers the services of an experienced multi-disciplinary outreach team. The team is comprised of four professional case managers who specialize in aging, mental health, and substance abuse concerns.

The Harvest Project case management services are cost-free, confidential and include:

  • Identification and Screening
  • Crisis Intervention
  • Professional Consultation
  • Information and Referral
  • Personal Advocacy
  • Assessment
  • Collaborative Service Planning
  • Long term follow up

Service Population:
Vulnerable residents of Lancaster County, age 55 and older, who have significantly diminished personal abilities to cope with the life circumstances they face. These individuals may not be able to manage their own safety and well-being. They are victims of the combined effects of health impairments, mental illness, and substance abuse. Their condition may be further aggravated by the disruption or absence of social and environmental supports. Characteristically, these older persons present issues too complex to be adequately addresses by any single human service agency
