Nutrition Assessments & Consultation Services
Aging Partners provides Nutrition Consultation to clients who participate in the Older Americans Act sponsored meal services. Annual nutrition clinics are scheduled at all centers in the service area. Nutrition assessment, intervention plans, counseling, coordination with caregivers and case workers and follow-up are available to clients who screen at risk. Nutrition Consultation is also provided to older adults who are referred by other OAA service providers in the eight-county service area. Service is provided at no cost to eligible older adults.
